I understand that my child as a participant in any event hosted by AIM Sports Academy will be responsible for his/her conduct, proper use of the equipment and facilities. I agree that the sponsors of this event, staff members or facility holder will not be held responsible legally or financially liable for any loss of personal properly, bodily injuries, or the result thereof incurred and suffered by the applicant and members of their family.

I hereby grant permission to Athletes in Mission to photograph or video record my child as they participate in any Athletes in Mission event. I also hereby grant permission to Athletes in Mission to use those images in promotion of future Athletes in Mission events. By signing this release I understand this signifies that photographic or video recordings of my child may be electronically displayed via the internet or in a public setting.
